Four-chamber delayed-phase cardiac magnetic resonance image obtained at the mid-diastolic phase shows an apical aneurysm with intracavitary thrombus (arrowheads) and diffuse subendocardial delayed hyperenhancement of the LV septum and lateral wall (arrows), consistent with old MI.
