A. Although front-line thrombectomy can be successful in achieving primary reperfusion in ICAS-related ELVO, instant reocclusion can frequently occur because remaining endothelium is irritable due to rethrombosis. B. Intraarterial use of glycoprotein IIb/IIIa inhibitors, such as tirofiban, can stabilize irritable endothelium. C. Emergent angioplasty with or without stenting can treat stenosis and is another viable option for preventing reocclusion.
