Reduced model included factors with p value < 0.2 in univariate logistic analysis.
The final model was based on bootstrap analysis using forward stepwise logistic regression with an entry criterion of p value < 0.20 and a retention criterion of p value <0.05. The final model provided a very good fit of the data (Hosmer-Lemeshow goodness-of-fit test, p = 0.94).
