*Total sample invited to participate in survey was selected by random selection among panel members. **Exclusion prior to survey included exclusion for any one of exclusion criteria. ***Exclusion due to response included: quit survey, provided their answers to a series of questions in the same place on a rating scale; completed the study in an unrealistically short time (i.e., less than 40% of the mean time taken by patients and HCPs); or provided nonsensical open-ended responses. ****A maximum requested sample was 250 patients per country (with a minimum of 50 patients in the acute, post-acute, and remission phase), and 125 HCPs per country. Patients and HCPs who signed up after maximum was reached were excluded from the survey.
