All models included maternal age, maternal asthma, maternal diabetes, premature rupture of membranes, maternal tobacco status, and robust sandwich variance estimators for lack of independence among sibling births. For all models, stratification was used for 10-year epoch of birth and maternal tobacco status to avoid violating the proportional hazard assumption.
