How would you rate your communication skills with patients? 1 (poor) to 10 (excellent)
How convinced are you that it is important to use Teach Back (ask patients to explain key information in their own words)? 1 (not at all important) to 10 (very important)
How confident are you in your ability to communicate with patients from all backgrounds, cultures, and languages? 1 (not at all confident) to 10 (very confident)
How often do you ask patients to explain what they need to know or do to take care of themselves? 1 (never); 2 (some patient visits); 3 (many patient visits); 4 (all patient visits)
How would you rate these elements of communication with patients?
  • Use a caring tone of voice and attitude

  • Use plain language

  • Ask the patient to explain, in his or her own words, what they were told

  • Use open-ended questions

  • Avoid asking questions that can be answered with a yes or no

  • Take responsibility for making sure you were clear

  • Explain and check again to see if the patient is unable to explain his or her care plan
