Low melanoma incidence area/population Moderate to high melanoma incidence area/population
  • Two (synchronous or metachronous) primary melanomas in an individual and/or

  • Three (synchronous or metachronous) primary melanomas in an individual and/or

  • Families with at least one invasive melanoma and one or more other diagnoses of melanoma and/or pancreatic cancers among first- or second-degree relatives on the same side of the family

  • Families with at least one invasive melanoma and two or more other diagnoses of invasive melanoma and/or pancreatic cancer among first-or second-degree relatives on the same side of the family
