Example of a refeeding process in terms of macronutrient and micronutrient intake [19].

Days Recommendations
Day 1–3
  • Start with 10–15 kcal/kg (600–1000 kcal/day).

  • Prophylactic electrolyte supplementation (P, K, Mg).

  • Thiamine (200–300 mg/day).

  • Vitamins (200% RDI).

  • Minerals and trace elements (100% RDI).

  • Restrict the contribution of fluids to a zero balance.

  • Restrict sodium to <1 mmol/kg/day.

  • Glucose and electrolyte levels and the appearance of oedema should be adequately monitored, since the highest risk of RS occurs in these early days.

Day 4–10
  • Calorie intake will increase to allow weight gain, continuing with electrolyte, vitamin and mineral supplementation and close monitoring.
