Inclusion criteria Exclusion criteria
1. Hospitalized at UI Health 1.Unable to understand and/or speak English
2. Aged 18 years or older 2. Unable/or declined to give informed consent
3. Index admission physician diagnosis of:
  • -

    Heart failure,

  • -

    Pneumonia,

  • -

    Chronic obstructive pulmonary disease,

  • -

    Myocardial infarction, or

  • -

    Sickle cell disease

 3. Previous participant in the PArTNER study*
4. Planned transfer to another acute care facility
5. Planned discharge to facility other than home (e.g., long term care facility)
6. Receiving hospice care or planned to be discharged to hospice care
7. Plan to leave against medical advice
