Surveillance care | Surveillance tests are recommended to detect cancer recurrence. Tailored reminders about guideline-concordant surveillance care were delivered to the patient. A table also summarized tests completed and the next test due. |
Community resources | Links to cancer survivor information resources and support groups. |
Relationships | Patients identify role-based individuals (provider, caregiver) with whom to share the personal health record. Relationships are configurable to allow access to part or all components of the personal health record. |
Journal | Patients enter unstructured information about their experience with cancer and treatment into a journal (using My Relationships; others with whom the journal is shared can write in-line comments) |
Secure messaging | Enables patients to send and receive messages from individuals with whom they have created a relationship. |
