  • INR 2–4: 25 IU/kg by IV push

Strategy 2: INR-Based Dosing
  • INR <5: 500 units; INR ≥5: 1000 units

Strategy 3: Fixed Dose
  • 1500 IU

INR-based dosing is most effective with 3-factor preparations.
Absolute dosing strategies should not be used with 3-factor PCCs.
Any of the 3 strategies can be used with 4-factor PCCs.
