History Pain in left leg for 2 weeks after physical exercise. 10 months of fever (afternoon37.5 ℃ to 38.5 ℃ for 3-4 days with an interval of5-7 days, spontaneous remission) with fatigue and mild pain of bilateral thighs. Left lumbosacral pain for 4 months.
Physical examination Deep tender pain in the upper section of left leg without swelling and palpable mass. Unremarkable. A firm, immobile mass of8 cmx7 cm at the left sacroiliacjoint with tenderness radiating to the left leg. The mass was warm and well-demarcated without swelling and erythema.
Radiological findings MRI: oval mass of 4.5cmx 2.5cmx2.6 cm in left upper femur, partial cortical osteolysis. PET-CT: negative findings (Figure 1) PET-CT: enhanceduptake of 18F-FDG in the marrow of left upper femur and lower tibia (Figure 2). CT and X-ray: a osteolytic mass of 9.6 cm x7.8 cmx6.4 cm at the left sacroiliac joint, projected into pelvis, presence of inner bone fragments, absence of necrosis (Figure 3).
Laboratory findings Neutrophils 3190/μL, lymphocytes 2440/μL, monocytes 340/μL, eosinophils 140/μL, basophils 40/mL, RBC 4.86×106/μL, Hb 163g/L, hematocrit 0.459, MCV 94.4fl, MCH 33.5 pg, platelets 1.57×105/μL, MPV 11.1fl. Neutrophils 6570/μL, lymphocytes 2370/μL, monocytes 510/μL, eosinophils 420/μL, basophils 120/μL, RBC 4.32×106/μL, Hb 130g/L, hematocrit 0.382, MCV 88.4fl, MCH 30.1pg, platelets 1.3×105/μL, MPV 10.2fl. Neutrophils 6990/μL, lymphocytes 2580/μL, monocytes 420/μL, eosinophils 50/μL, basophils 10/μL, RBC 3.46×106/μL, Hb 101g/L, hematocrit 0.297, MCV 85.8fl, MCH 29.2pg, platelets 1.73×105/μL, MPV 9.0fl.
Normal blood cell count and morphological analysis. Negative for BCR-ABL1 in fluorescence in situ hybridization and JAK2 mutations. Normal in chromosome examination of peripheral blood cells. No evidence of anemic disorders, renal and hepatic dysfunction, infections, autoimmune disease, cancer-related antigens, lymphoma or leukemia.
Medical intervention Remove the mass. Remove the mass. Tumor biopsy.
