Numbers were retrieved from May 2017 to February 2018 in the University Hospital Münster. From May 2017 to November 2017, the form was pilot tested. In November 2017 the first evaluation of the previously existing system was performed and the form was used in clinical routine. The second evaluation of the new system was performed in January.
