(A)The Logistic Regression model uses the Neuro-QoL Fatigue t-score (Qol Fatigue), the Physical Functioning score (PF) and the SF-36 Mental Health component score (MCS) to calculate a logistic predicted probability of post-treatment Lyme disease symptoms (PTLDs). The equation is: $logit\left(\text{Pr}\left(Y=PTLDs\right)\right)=49.66+0.14\times \left(QoLFatigue\right)-0.24\times \left(PF\right)-0.76\times \left(MCS\right)$. (B) The Decision Tree model categorized QoL Fatigue t-scores ≤ 42.8 into the recovered group and QoL Fatigue t-scores > 42.8 into the post-treatment Lyme disease symptoms (PTLDs) group.
