Follow-up data at 6 mo are missing for 69 participants (25 in the patient-provider group, 26 in the patient group, 11 in the provider group, and 7 in the usual care group) and at 12 mo for 102 participants (37 in the patient-provider group, 31 in the patient group, 16 in the provider group, and 18 in the usual care group).
Data for the total and physical function scores are missing for 3 participants (1 at baseline, 1 in the patient group at 6 mo, and 1 in the provider group at 12 mo).
