Active case-finding programs: Arizona, Arkansas (2012–2015), California, Delaware, Georgia (Metropolitan Atlanta Congenital Defects Program), Iowa, Louisiana, Massachusetts, Minnesota, North Carolina, Oklahoma, Puerto Rico, South Carolina, Texas, Utah.
Cases were included when gestational age was greater than or equal to 20 completed weeks gestation. For select states, ascertainment area differed from that reported in the supplemental tables. Totals include unknown/other.
