Covariates included rural location, medical school affiliation, system affiliation, ownership status, size, safety-net status, the number of full-time equivalent registered nurses per 1,000 patient-days, the percentage of discharges reimbursed by Medicare, the percentage of county residents living below the poverty line, and the percentage of county residents that are Black.
