Inclusion and exclusion criteria

Inclusion criteria

>18 years of age LVEF 36–50% on echocardiography, CMR, or Multigated Aquisition Scan (MUGA) in the previous 6 months Coronary artery disease or dilated nonischemic cardiomyopathy (idiopathic or familial) On optimal and maximally tolerated doses of beta‐blockers or ACE inhibitors (or angiotensin receptor blockers if intolerant to ACE inhibitors) Reasonable likelihood of compliance with the study procedures and follow‐up requirements

Exclusion criteria

Patients who are pregnant Standard CMR contraindications Current pacemaker or defibrillator in situ Renal impairment defined as an eGFR <30 milliliters/min Recent myocardial infarction (<40 days) Percutaneous coronary angioplasty or CABG within 90‐days NYHA class IV Life expectancy <12 months Current indication for device therapy
