Table 1.

Patient and process composite scores

Variable Maximum score Target
HbA1c 2 (4 × 0.5) 0.5 point for each quarter that HbA1c is measured in the previous year
Blood pressure 2 (4 × 0.5) 0.5 point for each quarter that blood pressure is measured in the previous year
LDL cholesterol 1 If LDL cholesterol is at least one time measured in the previous year
Microalbuminuria 1 If microalbuminuria is at least one time measured in the previous year
A prescription of statin 1 If a statin is prescribed in the previous 6 months
A prescription of aspirin/clopidogrel 1 If aspirin or clopidogrel is prescribed in the previous 6 months
A prescription of ACE-inhibitor/sartan if antihypertensive drugs are prescribed 1 If antihypertensive drugs are prescribed in the previous 6 months, ACE-inhibitors or sartans are recommended. If systolic blood pressure > 130 mmHg or if diastolic blood pressure > 80 mmHg, the patient is diagnosed with hypertension.
