 Setting: in hospital at start of study, community follow up. Diagnosis: people with schizophrenia or related disorders.

 Sex: men & women. 
 History: people in their first episode reported separately. 1. Compliance therapy: administered according to manual of Kemp and David, given over 5 sessions of 30‐60 minutes. N=150. 
 2. Non‐specific' counseling: delivered over 5 sessions of 30‐60 minutes duration. 
 N=150. Service use: bed occupancy (primary outcome). 
 Compliance: clinical interview. 
 Other routinely recorded measures of mental state, quality of life, general functioning, adverse effects and service use. * Powered to be reasonably confident of finding a 10% difference between groups for the primary outcome.
