| • Symptomatic bleeding in a critical organ (e.g., ocular, pericardial, medullary) |
| • High risk of bleeding |
| • Frailty/frequent falls |
| • Comorbidities (e.g., intestinal angiodysplasia, renal failure, blood dyscrasia) |
| • Lack of adherence to treatment |
| • Labile INR |
