Trimester‐specific data and overall BMI and fasting plasma glucose require month of pregnancy information, which was not available in 2013 to 2014. For 1999 to 2006, calculated on the basis of annual income (INDFMPIR); for 2007 to 2014, calculated on the basis of monthly income (INDFMMPI).
