Pancreatic stellate cell activation and islet fibrosis in type 2 diabetes mellitus (T2DM). High‐glucose levels, insulin and angiotensin II (Ang II), and a release of pro‐inflammatory cytokines induce pancreatic stellate cell (PSC) activation. Activated PSCs cause extracellular matrix production and cell proliferation. These phenomena appear to drive fibrosis within the pancreatic islets in type 2 diabetes mellitus. Islet fibrosis can be detected by immunostaining for alpha‐smooth muscle actin (α‐SMA) and trichrome.
