Interventions | General inhalational anaesthesia with fentanyl, nitrous oxide, enflurane and pancuronium bromide for all participants with or after the surgery Thoracic epidural analgesia (n = 22): at T9‐T10 or T10‐T11 loaded with bupivacaine for a sensory level of T4 and epidural morphine for at least 48 hours postoperatively Morphine IV PCA (n = 20) |
