First-line agents for standard first-line anti-tubercular therapy

Isoniazid 5 mg/kg Give pyridoxine 10 mg daily to prevent peripheral neuropathy; if established peripheral neuropathy, give pyridoxine 50–75 mg daily
Protect tablets from light
Rifampicin 10 mg/kg Preferably given 30 min before food
Pyrazinamide 25 mg/kg Avoid with active hepatic disease (see further discussion on drug-induced hepatitis, below); adjust dose for renal function
Ethambutol 15 mg/kg Adjust dose for renal function
Ocular examination to be done before and periodically during treatment
