  • Intensive care unit admission

  • Failure of outpatient antibiotic therapy

  • Radiographic cavities

  • Leukopenia

  • Active alcohol abuse

  • Chronic severe liver disease

  • Severe obstructive/structural lung disease

  • Asplenia

  • Recent travel (within past 2 weeks)

  • Positive Legionella UAT result

  • Positive pneumococcal UAT result

  • Pleural effusion
