Each of the quantile regression models were stratified by sex and include diabetes (yes/no), smoking (yes/no), height (in), birth cohort (1930 or earlier, 1931–1940, 1941–1950, 1951 or later), age (years) and CRF (ml O2·kg−1·min−1) and a two-way interaction between diabetes and CRF (diabetes*CRF) as predictors. The referent level for birth cohort is the group born 1930 or earlier.
