A, Diagrammatic representation of anatomic structures involved with transtracheal aspiration technique. The best landmark for percutaneous puncture is the cricothyroid ligament of the larynx, although the tracheal lumen also can be entered between cervical tracheal rings. B, The needle is advanced and directed slightly caudad until the trachea is entered. Once the needle is positioned within the tracheal lumen, the catheter is advanced through the needle and down the trachea.
