An 86-year-old man with a post-TURBT VI-RADS 5 infiltrative lesion in the anterior bladder wall, presenting evident extravesical extension on T2WI (A,B), early enhancement extending to the extravesical fat (C), and high signal intensity, also extending to the extravesical fat, on DWI (D). Although most of the intravesical vegetative lesion was removed during TURBT, an extensive lesion persisted, allowing the characterization of MIBC. Histopathological analysis of the lesion indicated invasive high-grade papillary urothelial carcinoma with invasion of subepithelial tissues and infiltration of muscularis propria.
