Participants | Inclusion: > 18 years admitted to hospital and meeting at least 1 of the following: ≥ 5 medications, ≥ 3 changes to medication in hospital, past history of medication‐related problems, patient referred to medicines management clinic service by hospital doctor or pharmacist due to concerns about ability to manage medicines |
Interventions | Intervention: medicines management clinic within an outpatient setting as well as follow‐up telephone calls from a clinical pharmacist (extension of ongoing integrated medicines management programme (IMMP)) |
Outcomes | Medication adherence (using medication adherence report scale and beliefs about medicines questionnaire), time to re‐admission to hospital, number of re‐admissions, number of GP consultations/home visits, number of accident and emergency visits, health‐related quality of life, cost utility analysis |
