Methods RCT
Participants Inclusion: > 18 years admitted to hospital and meeting at least 1 of the following: ≥ 5 medications, ≥ 3 changes to medication in hospital, past history of medication‐related problems, patient referred to medicines management clinic service by hospital doctor or pharmacist due to concerns about ability to manage medicines
Interventions Intervention: medicines management clinic within an outpatient setting as well as follow‐up telephone calls from a clinical pharmacist (extension of ongoing integrated medicines management programme (IMMP))
Outcomes Medication adherence (using medication adherence report scale and beliefs about medicines questionnaire), time to re‐admission to hospital, number of re‐admissions, number of GP consultations/home visits, number of accident and emergency visits, health‐related quality of life, cost utility analysis
