The following covariates were included in the multivariate logistic regression: age, sex, race/ethnicity, education, employment status, marital status, reported health status, family income, own/rent resident, MSA status, region, presence of others, location of interview, number of contact attempts, case reassigned to different interviewer, householder(s) expressed privacy concerns, and householder(s) expressed time-related concerns.
