Box 3 is the “Document System Scoring Report”, where documentation for follow-up items can be entered. Box 4 is part of the AMS accordion report which contains vital signs, fever curve, intake/output, relevant hematology and chemistry laboratory values, microbiology, radiology, medications and allergies.
