Please note that (1) the 95% confidence intervals of the VoL were estimated using the Delta method (Daly et al. 2012); (2) the parameters θ2 and φ2 have no t-values, because they were not estimated but calculated as differences of other parameters: θ2=Θ-θ1 and φ2=Φ-φ1; (3) the parameter Θ is not estimated because it is set to one for normalisation, i.e., all other parameters are estimated in units of Θ
