A template is created, applied to the first patient of the sample and modified until the produced plan is clinically acceptable. Then the template is applied to the second sample patient: if the resulted plan is clinically acceptable, then the template is applied to further patients. Otherwise it is modified as requested and then applied to further patients (including the first). Mainly the template is optimized by iteratively changing the cost functions or the isoconstraints.
