*Adjusted ORa—obtained using a hierarchical logistic regression model that adjusted for AMG risk score, sex, Index of Multiple Deprivation score, previous acute myocardial infarction, angina, peripheral vascular disease, cerebrovascular disease (stroke), percutaneous coronary intervention, coronary artery bypass grafting, chronic renal failure, diabetes, smoking status, hypercholesterolemia, hypertension, asthma/chronic obstructive pulmonary disease, family history of coronary heart disease, annual hospital PPCI volume and month and year of admission. Hospitals included as random intercepts (46 ‘24/7’ interventional centres included).
