Develop strong relationships with patient, family, clinician, and primary care practice partners to increase comfort with SICP and ACP conversations. Document implementation barriers and facilitators from multiple perspectives to encourage replication and spread. Explore ways of engaging patients, families, and PCCs and practices in this conversation outside study parameters (ongoing communication). |
