Cumulative air kerma (dose) at the interventional reference point  > 5 Gy or 5000 mGy
Measure peak skin dose  > 3 Gy or > 3000 mGy
Number of cine acquisition series  > 20

Patient had multiple procedures within the last 12 months
Wrong procedure performed
