After initiating the V-Y quadricepsplasty, tension is held distally to achieve the appropriate amount of tendon mobility to reach the superior pole of the patella. Once acceptable excursion is obtained, the proximal Y-limb (blue bars) is sutured together (double arrows) in a side-to-side fashion to allow for distalization of the tendon.
