Table 5. Principles of mass casualty triage.

Tag color Triage Assessment Disposition
Red Highest priority; these patients have life-threatening injuries Spontaneously breathing when airway-opening maneuvers performed; respiratory rate >30/min; radial pulse absent; capillary refill >2 sec; unable to follow verbal commands Immediate attention required
Yellow Second priority; these patients may have life-threatening injuries but are not imminently deteriorating Spontaneously breathing; respiratory rate <30/min; radial pulse present; capillary refill <2 sec; obeys verbal commands Care may be delayed for now. Status not expected to deteriorate over several hours
Green Third priority; these patients have non-life-threatening injuries Able to walk and remove themselves from the scene (“walking wounded”) Care may be delayed as status is not likely to deteriorate
Black Lowest priority; these patients are unlikely to survive given the severity of the injuries and/or the level of care available Not spontaneously breathing despite attempts to position the airway Provide analgesia and other palliative measures
