Severe, concentric biventricular hypertrophy with granular sparkling appearance of the myocardium, thickened papillary muscles, bi-atrial enlargement and pericardial effusion. The regular size of the aortic root and the ascending aorta should have called into question the initial diagnosis of hypertensive heart disease.
