The trocar is placed in the left nostril for the passage of instrumentation. The right nostril has rubber tubing taken from a Lukens tube, which will be used to pass the endoscope and ensure an adequate seal C. Placement of endoscope in the right nostril and high-speed drill through the trocar in left nostril D.
