A 79-year-old female diagnosed with metastatic Hurthle cell carcinoma presents radioactive iodine (RAI)-resistance. Initial FDG PET/CT shows increased metabolic activity in the recurrent metastasic right level II/III cervical chain lymph nodes (white arrows) in 2016. Six months after RAI therapy, follow-up FDG PET/CT in 2017 shows increased size and metabolic activity in the right level II/III cervical chain lymph nodes, indicating RAI-resistance.
