§Responses to individual items assigned to the Making Referrals scale are on a 5-point frequency scale from 1 (Never) to 5 (Always). Responses to items assigned to the other scales are on a 7-point frequency scale from 1 (Never) to 7 (Always). All items ask about experiences in the last the last 3 months. Scale scores are calculated using data from respondents whose data are non-missing on at least half of the items in that scale. The scales are scored as the simple average of the scores for the items assigned to that scale and have a theoretical range from 1 to 5 (Making Referrals scale) or 1–7 (for the rest of the scales)
