Variables were coded as follows: Gender: 1 = Male; 2 = Female; Income: 0 = Child not eligible for free or reduced price school meals; 1 = Child eligible for free or reduced price school meals; Insurance: 0 = No insurance; 1 = Private or Public insurance. Age and education were treated as continuous variables.
