Severe AS was defined as an AVA ≤1.0 cm2 and a mean gradient ≥40 mmHg. Normal LV outflow was defined as a stroke volume index >35 mL/m2 and a mean transvalvular flow rate >210 ml/s. These data were prospectively collected in 3 centers: QHLI, Bichat Hospital, and Mayo Clinic (31–34).
