Immature respiratory drive, airway abnormalities, immature lungs stiff from surfactant deficiency, inflammation or fibrosis, pliable rib cage and chest wall, and pulmonary over-circulation with edema from a left to right shunt such as patent ductus arteriosus (PDA) contribute to extubation failure.
