Keys to Arthroscopic Posterior Femoroplasty With Anteriorly Based Pre-bent Hip Burrs

Step Procedure Rationale
1 Central compartment hip arthroscopy
Perform acetabuloplasty and labral repair, if indicated
  • Diagnostic arthroscopy to determine whether acetabular pathology is present

2 Release traction device, flex hip to ∼40°
Peripheral compartment entered and traction sutures applied (anteriorly and laterally)
Anterolateral cam resection
  • Improve visualization

  • Treat anterolateral cam lesion

3 Place hip in flexion and maximal external rotation (figure-of-4 position)
70° arthroscope through anterolateral portal with pre-bent burr in mid anterior portal
  • Maximal rotation around femoral head to expose and visualize posterior cam lesion for resection

  • As resection occurs, more external rotation can be achieved

  • Remain proximal to the posterolateral retinacular vessels

4 Dynamic and fluoroscopic testing post-resection
  • Determine if posterior cam lesion has been adequately decompressed
