In situ carcinoma is characterized by abrupt hyperplasia and dysplasia of the epidermis and outer root sheath (green arrows; also see inset). Squamous cell carcinoma is characterized by invasive anastomosing islands and trabeculae of neoplastic epithelial cells (black arrow) with multiple keratin pearls.
