• No good soft-tissue envelope (i.e. presence of fistula)
• No good bone stock
• No identified, nor susceptible pathogens
• septic patients
• prior failed septic revision arthroplasty
• immunosuppression
• body weight >100 kg or BMI >30
• No oral antibiotics with good bioavailability
• infection of neurovascular bundle
• isolation of enterococcus or streptococcus species
