Over 1 year, the percentage of moderate-/high-risk patients with (A) HbA1c <8% and (B) LDL <100 mg/dL increased significantly in the CMC-TI group (+26.8%, P = 0.001 and +14.2%, P = 0.045, respectively) but not in usual care (+8.7%, P = 0.172 and +3.9%, P = 0.401). Between-group differences at month 12 were statistically significant for HbA1c <8% (P = .005) and LDL-C <100 mg/dL (P = .043). Total N = 312.
